Cancellation12 hours for classes, 24 hours for massage.
- We require at least 12 hours notice for class cancellations.
- Massage and body treatment cancellations require at least 24 hours notice.
- Late cancels or no-shows will forfeit the class or session credit.
- Cancellations must be made via the booking platform.
ArrivalArrive 5–10 minutes early. Doors lock at class start.
- Please arrive 5–10 minutes early to settle in.
- Doors lock at class start time. No late entry, to protect the experience for others.
Class guidelinesWaiver, grip socks, clean equipment, women only.
- A signed health waiver is required before your first class. This can be completed online ahead of time or upon arrival at the studio.
- Grip socks are required for all reformer classes.
- Clean equipment after use using the provided wipes.
- Our women-only policy is strictly enforced. Please respect our safe space and honor the comfort and religious and cultural values of many of our clients.
WaitlistJoin the waitlist when a class is full.
- If a class is full, you may join the waitlist.
- You’ll be notified via email or text if a spot opens up. Make sure notifications are on.
Packages & expirationsNon-refundable, non-transferable.
- All class packages are non-refundable and non-transferable.
- Class packs expire based on the timeline listed at purchase.
Studio etiquetteShoes off, phones away, be kind.
To maintain a clean and calming environment at The Form:
- Please remove your shoes upon entering the boutique.
- Please silence phones and store them away during class.
- Be kind, courteous, and supportive. We are a community.
Policies & payment termsBy appointment only. All purchases are final.
- All sessions, classes, and services are by appointment only. Walk-ins are not accepted.
- All purchases are final. This includes drop-in classes, private sessions, duet sessions, packages, memberships, and merchandise. Once purchased, no refunds or transfers will be issued for any reason.
